This looks good to me. One comment on the Field Name - CARA Rating is correct, but I think we at least need a note that this is also known as “MPAA Rating.” It could be argued that MPAA would be the more familiar name to use here as the main name given that is what appears on the previews and on movie posters, but could go either way. Hopefully some others will voice opinions here!
